Medical Info Carrier
Shortly after the CycleGadgets.com Web site went on-line in July 2000, we came up with the idea of providing free Emergency Information Tags to motorcyclists. We worked up a Web form where riders could type in all the pertinent information, and then we'd print up a double-sided tag, laminate it, and mail it to the rider. The program was phenomenally successful. We sent out thousands of tags, and articles appeared in many magazines showing the tags and encouraging riders to wear them. Unfortunately, there were a couple of problems with the program. First off, since the tags had to be created in bulk, riders would often wait 6 weeks or more for their tag. Second, the process of collecting information and printing it for the rider was a concern to many people. Our privacy policy ensured that all medical information was deleted from our system once the tags were printed, but it still was a concern. Finally, the Web form we used turned out to be unreliable, and would occasionally simply stop taking requests with no warning or error message. Information was simply lost.
http://cyclegadgets.com/images/MICS_In_Helmet.jpgWe intended to bring in a programmer to beef up the Web form, but that would not address the other concerns. Then, we found the MICS (Medical Information Carrier System). The MICS is a plastic information carrier that adheres to the outside of your helmet, along with a two-sided information form that you fill out with medical information. Since you fill in the information yourself, there is no privacy concern regarding putting medical information into a Web form. Also, since no customization is needed, you'll get your tag in a matter of days rather than weeks.
The Fine Print: Limit one free MICS per customer per month. Additional MICS tags may be purchased for $1.99 each. International orders will be charged postage (Air Mail); please provide a credit card at checkout for that purpose. We reserve the right to cancel any orders that do not appear to be legitimate.
A special note regarding our Shopping Cart:Even if the only item you are ordering is a free MICS, due to limitations in our shopping cart you must still specify a method of payment. If you aren't purchasing anything else, you can simply choose "Check/Money Order" and make up a check number. We will process your order immediately; there is no need to send or FAX anything in.

If I didn't already have dogtags, I might consider this thing, it doesn't look like a bad idea.

Not trying to be rude but who do they call based on your tag info?

It fattoms me that you would order this online than just creating one yourself.
Use your computer and create all the pertinent info and then some.
Get a plastic pouch and use double sided tape
